Students at the School of Public Health in HPA 583b (Methods in Health Services Research) are conducting a quick online survey for the Yale community. The purpose of this research study is to examine general opinions, habits, and behavior related to a broad selection of topics ranging from exercise to health policy. 

Participation in this study will involve completing one or both of the online surveys.  Each survey will take approximately 10 minutes to complete depending on your Internet connection speed.  If you choose to complete both surveys, you will need to complete the short set of demographic questions twice.

These online surveys are anonymous. No identifying information will be collected and all findings will be analyzed in aggregate form.    In addition, this project has been approved by the Yale Human Subjects Committee.  Participation in this study is completely voluntary.  You are free to decline to participate, to end participation at any time for any reason, or to refuse to answer any of the questions.
